About the Role

We are seeking a highly motivated and compassionate Patient Advocate / Market Development Representative to join our growing team. In this role, you will be the first point of contact for potential patients, introducing them to the life-changing benefits of our chronic care management programs.

You will engage with patients through various channels (phone, email, etc.) to build rapport, understand their needs, and explain how our programs can help them better manage their conditions and improve their overall well-being.

Responsibilities:

  • Contact potential patients through various channels to introduce them to Welby Health's chronic care management programs
  • Effectively communicate the benefits of our programs and address patient questions and concerns
  • Qualify potential patients for program eligibility
  • Schedule consultations for patients with our clinical team
  • Accurately document patient interactions and maintain detailed records in our CRM system
  • Meet or exceed daily/weekly/monthly performance targets for patient outreach and enrollment
  • Stay up-to-date on the latest developments in chronic care management and healthcare regulations


Qualifications:

  • Excellent communication, interpersonal, and active listening skills
  • Strong ability to build rapport and establish trust with patients
  • Proven ability to handle objections and address concerns effectively
  • Experience working with medical benefits and explaining healthcare programs
  • Comfortable making a high volume of outbound calls
  • Proficient in using CRM systems and other relevant software
  • Passion for helping patients and improving healthcare outcomes
  • Prior experience in an SDR/MDR role is highly preferred
  • Medical licensing (e.g., MA, CNA) is a plus
